HOSPICE INTERDISCIPLINARY TEAM
The Hospice Interdisciplinary Team consists of a group of people committed
to providing quality care to patients and families. Their support is available
24 hours a day seven days a week.
ATTENDING PHYSICIAN
The patient's attending physician continues to manage the patient's
care.
HOSPICE MEDICAL DIRECTOR
The Medical Director oversees the treatment being provided by the hospice
team. They also coordinate with the attending physician to provide optimal
care.
HOSPICE NURSE
Licensed professional nurses will visit intermittently to assess the
patient's medical status, teach the family how to provide care and contact
the physician for symptom control.
SOCIAL WORKER
Hospice Social Workers offer emotional support and assist to coordinate
resources for patients and families.
CHAPLAIN
The Hospice Chaplain assists in identifying spiritual concerns.
HOME HEALTH AIDE
Home Health Aides assist the patient with personal care.
BEREAVEMENT COUNSELOR
The Bereavement Counselor supports family members through monthly letters
and articles pertaining to the grief process and provides both grief support
groups and individual grief counseling.
PHARMACIST
Pharmacists work with the hospice team to ensure the appropriate monitoring
and supervision of medications.
VOLUNTEER
Trained Volunteers provide a variety of services such as shopping and
visitation to give the caregiver needed relief.
OTHER THERAPISTS
In order to provide comfort care, dietitians, physical therapists and
other disciplines ordered by the physician may be utilized.
